Vitreous Separation
Definition
Vitreous separation, more formally called posterior vitreous detachment (PVD), occurs when the vitreous gel inside the eye separates from the retina. The vitreous is a clear, gel-like substance that fills the center of the eye and is attached to the retina by tiny fibers. As people age, the vitreous naturally shrinks and pulls away from the retina.
PVD is a very common age-related condition and is usually not sight-threatening by itself. However, because it can sometimes cause retinal tears or retinal detachment, any new symptoms should be evaluated promptly.
Causes and Risk Factors
Causes
The most common cause is the natural aging process. Over time:
- The vitreous becomes more liquid.
- Collagen fibers within the vitreous clump together.
- The vitreous shrinks and pulls away from the retina.
Eventually, the vitreous separates from the retinal surface, resulting in a posterior vitreous detachment.
Risk Factors
You may be at increased risk if you:
- Are over age 50
- Are highly nearsighted (myopic)
- Have had cataract surgery or other eye surgery
- Have experienced eye trauma or injury
- Have eye inflammation (uveitis)
Symptoms
Many people first notice symptoms suddenly.
Common Symptoms
Floaters
The most common symptom is a sudden increase in floaters, which may appear as:
- Black or gray specks
- Cobwebs
- Threads
- Rings
- Spots drifting across vision
These occur because clumps within the vitreous cast shadows on the retina.
Flashes of Light
Patients may notice:
- Brief flashes of light
- Lightning-like streaks
- Peripheral flashes
These flashes occur when the vitreous tugs on the retina during separation.
Symptom Progression
- Symptoms are usually most noticeable during the first few weeks.
- Floaters often become less noticeable over several weeks to months.
- Most patients gradually adapt to the symptoms.
Potential Complications
Although PVD itself is usually harmless, it can occasionally cause serious retinal problems.
Retinal Tear
As the vitreous pulls away, it may tear the retina.
Retinal Detachment
A retinal tear can allow fluid to accumulate under the retina, causing retinal detachment, which is an emergency.
Macular Hole
The pulling force can occasionally create a hole in the macula.
Epiretinal Membrane (Macular Pucker)
Scar tissue may develop on the retinal surface after vitreous separation.
Warning Signs Requiring Immediate Attention
Contact an eye doctor immediately if you experience:
- Sudden onset of many new floaters
- New flashes of light
- A gray curtain or shadow over vision
- Loss of peripheral vision
- Sudden decrease in vision
These symptoms may indicate a retinal tear or retinal detachment.
Diagnosis
Dilated Eye Examination
The primary diagnostic test is a comprehensive dilated retinal exam. This allows the ophthalmologist to determine whether the vitreous has detached and whether any retinal tears or detachments are present.
Additional Imaging
In some cases, retinal imaging or ultrasound may be used if examination of the retina is difficult.
Treatment
Uncomplicated PVD
Most cases require no treatment.
- Floaters often become less noticeable with time.
- Flashes usually decrease as vitreous traction resolves.
- Regular follow-up may be recommended.
Treatment of Associated Complications
If retinal complications develop:
- Retinal tears may be treated with laser photocoagulation or cryopexy.
- Retinal detachment may require surgery.
- Macular holes or epiretinal membranes may require vitrectomy surgery.
Prevention
PVD is generally a normal age-related change and cannot be prevented. However:
- Routine eye examinations are important as people age.
- High-risk patients (high myopia, prior eye surgery, retinal disease) should maintain regular ophthalmic follow-up.
- Any sudden visual changes should be evaluated immediately.
