What to Expect
Please bring the following with you:
- Prescription glasses if you wear them
- Any eye drops you are taking
- All your insurance cards
- Driver’s License / Photo I.D.
- Insurance referral forms (HMO patients)
- Guardian or Power of Attorney authorization documentation, if applicable
- Medications – the containers or a list of all medications you are currently using
- Names and addresses of other doctors you are currently seeing
- Something to eat if you are diabetic
- A driver – to drive you home from your appointment because your pupils will be dilated
- Sunglasses – because your eyes will be sensitive to light while your pupils are dilated
