Vitreous Separation

Definition

Vitreous separation, more formally called posterior vitreous detachment (PVD), occurs when the vitreous gel inside the eye separates from the retina. The vitreous is a clear, gel-like substance that fills the center of the eye and is attached to the retina by tiny fibers. As people age, the vitreous naturally shrinks and pulls away from the retina. 

PVD is a very common age-related condition and is usually not sight-threatening by itself. However, because it can sometimes cause retinal tears or retinal detachment, any new symptoms should be evaluated promptly.

Retinal Tear

As the vitreous pulls away, it may tear the retina. 

Retinal Detachment

A retinal tear can allow fluid to accumulate under the retina, causing retinal detachment, which is an emergency. 

Macular Hole

The pulling force can occasionally create a hole in the macula. 

Epiretinal Membrane (Macular Pucker)

Scar tissue may develop on the retinal surface after vitreous separation.

Diagnosis

Dilated Eye Examination

The primary diagnostic test is a comprehensive dilated retinal exam. This allows the ophthalmologist to determine whether the vitreous has detached and whether any retinal tears or detachments are present.

In some cases, retinal imaging or ultrasound may be used if examination of the retina is difficult.
